HAI Floxuridine Dose Adjustment

Compares total bilirubin, AST and alkaline phosphatase against the reference D1 labs and applies the HAI floxuridine dose-modification table. Labs can be read from an Epic flowsheet screenshot or pasted text, or typed in by hand.

1 Load the labs

Type the nine values from the chart — total bilirubin, AST and alkaline phosphatase on the reference D1, the prior D15 and the current D1 — straight into the table in step 2. The dates are optional.

2 Dates and lab values — review every cell

After a hold, this stays the last D1 when floxuridine was given — not C1D1.

Prior D15 is suggested as the date closest to D1 + 14 days (±3), and current D1 as the latest date. Change either with the selector in the column header; “Other date…” takes a date that is not in the loaded labs.

Highlighted cells were unclear to the reader, flagged H/L, or disagreed between reads — check them against the chart. ALT is not used for floxuridine dosing.

Rules follow the Intera Oncology HAI program guide §4.2–4.8 with Moffitt laboratory normal ranges; institutional protocol, pharmacist verification and clinical judgment supersede this result. Screenshot reading can be wrong — every value must be checked against the chart before calculating.
